We will collect certain personally identifiable information about you (“Personal Information”) when you visit the Web Site, use end user software, or engage in other Apixio services that link to or display this Privacy Policy. We will not sell your Personal Information to anyone unless we have obtained your explicit consent. Information We Obtain from Third Party Sources Recruitment: In some instances, Apixio engages with third party sources to obtain additional information about you. For example, Apixio collects contact information from professional network intelligence companies or industry event organizers. Information collected by professional network intelligence companies is provided by you or is publicly available and used by Apixio’s talent acquisition team to determine your interest in and fit for employment with Apixio. Marketing: In some instances, Apixio may work with third parties to obtain additional information about you. For example, Apixio may receive business contact information from industry event organizers or business intelligence providers. This data is voluntarily provided by you or publicly available and used by the Apixio marketing and sales team to determine your company’s interest in Apixio’s products and services. You may opt out of marketing communications at any time by clicking the “unsubscribe” link in any email correspondence from Apixio. How We Use the Information We Collect Sharing Your Information: Third-Party Service Providers Security We are committed to ensuring that your information is secure. In order to prevent unauthorized access or disclosure, we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect online. Unfortunately, no data transmission or storage system can be guaranteed to be 100% secure. We further protect your information from potential security breaches by implementing certain technological security measures including encryption, firewalls and secure socket layer technology. How We Use Cookies A cookie is a small file which asks permission to be placed on your computer’s hard drive. Apixio does not knowingly collect personal data from Minors. If it comes to our attention that we have collected Personal Information from a Minor, we may delete this information without notice. If you have reason to believe that this has occurred, please contact Apixio at the email address listed below. “Do Not Track” California law requires us to let you know how we respond to web browser Do Not Track Signals (“DNT”). Apixio does not track its customers over time and across third party website to provide targeted advertising and therefore does not respond to DNT signals. Opt Out Apixio offers opt-out mechanisms for marketing communications. If you choose to opt out of marketing communications, you will be added to Apixio’s opt-out list as required by applicable law. Apixio will not send marketing communications to any e-mail address on the applicable opt-out list. In certain cases we may ask you for additional information for purposes of complying with applicable laws. We may also inquire about criminal records. We will do so only where permitted by applicable law. 2.2 Sensitive personal information With the possible exceptions of contact information, professional qualifications, application process information, job preferences, and employment history, all of the categories above include, or contain information from which it may be possible to infer, sensitive personal information. However, we do not use or disclose sensitive personal information in ways subject to the right of California residents to limit use of sensitive personal information under the CCPA. 2.3 Sources of personal information We collect personal information from you when you apply for a job and throughout the job application or recruitment process. We may also collect your personal information from other sources and combine it with the personal information you provide us. California privacy rights 4.1 Your privacy rights. California residents have the rights listed below under the CCPA. However, these rights are not absolute, and in certain cases we may decline your request as permitted by law. We may use the categories of personal information above for the following purposes: 4.2 How to exercise your California privacy rights You may submit requests to exercise your rights to Human Resources at hr@apixio.com, by meeting with a member of Human Resources, or by filling out Apixio’s CCPA Consumer Rights Request Form. Submitting your request through such channels allows us to verify your identity as required by the CCPA. As such, we cannot accept requests through other channels. We cannot process your request if you do not provide us with sufficient detail to allow us to understand and respond to it. We reserve the right to confirm your current California residency. Your authorized agent may make a request on your behalf upon our verification of the agent’s identity and our receipt of a copy of a valid power of attorney given to your authorized agent pursuant to California Probate Code Sections 4000-4465. If you have not provided your agent with such a power of attorney, you must provide your agent with written and signed permission to exercise your CCPA rights on your behalf, provide the information we request to verify your identity, and provide us with confirmation that you have given the authorized agent permission to submit the request. 5. Other information about this Notice 5.1 Third parties This Notice does not address, and we are not responsible for the practices of any third parties, which have their own rules for how they collect and use your personal information. Our links to third-party websites or services are not endorsements. 5.2 Changes to this Notice We reserve the right to change this Notice at any time. We need to verify your identity to process your requests to exercise your rights to know, access, deletion, and correction requests, and we reserve the right to confirm your California residency. To verify your identity, we may require authentication into your Service account, provide identifiers we can match against information we may have collected from you previously, confirm your request using the email or telephone account stated in the request, provide government identification, or provide a declaration under penalty of perjury, where permitted by law. Authorized agents. Your authorized agent may make a request on your behalf upon our verification of the agent’s identity and our receipt of a copy of a valid power of attorney given to your authorized agent pursuant to applicable state law. If you have not provided your agent with such a power of attorney, we may ask you and/or your agent to take additional steps permitted by law to verify that your request is authorized, such as information required to verify your identity and that you have given the authorized agent permission to submit the request. Notice of right to opt-out of the “sale or sharing” of your personal information. Like many companies, we use services that help deliver interest-based ads to promote our business and products. Our use of some of these services may be classified under California law as “selling or sharing” of personal information from which you have the right to opt-out.
